Appointments

Your scheduled appointment time has been reserved specifically for you. We request 24 hours’ notice if you need to cancel. We understand that unforeseen events sometimes require missing an appointment and we appreciate your cooperation. A $50-per-hour short-notice fee applies to appointments cancelled within 24 hours of the appointment time.

Insurance

Our office is committed to helping you maximize your dental benefits. Because dental insurance policies vary, we can only estimate your coverage in good faith and cannot guarantee coverage due to the complexities of insurance contracts. Coverage is subject to limitations, exclusions, waiting periods, frequency and age restrictions, deductibles, and maximums, which are your responsibility. Your estimated patient portion is due at the time service is rendered. As a service to our patients, we bill dental insurance companies on your behalf.

To process claims efficiently, our office and the insurance companies require: (1) the correct spelling of your name as on file; (2) your ID# and Group#; (3) your birthdate and Social Security Number; and (4) the correct mailing address on file. All claims are filed electronically and most process within 3 to 30 days. Dental insurance is a contract between you and your insurer; if an issue arises you may need to contact them directly.
